Step-by-Step New Patient Registration Procedure
1. After completing the pre-registration form at the bottom of the page, you will be notified by email of your acceptance into the practice within one week. Prospective patients should fully review the information on this website and in the Practice Policies Document. Any questions may be emailed to Dr. Speach at elissaspeach@onebox.com. 2. Dr. Speach will create your accounts with the online appointment scheduling program. You will receive a confirmation email from Dr. Speach with an invitation to schedule your new patient visits and instructions on how to do so. At this time, you should complete the New Patient Medical History This survey is completely secure and your information will be electronically encrypted for your privacy.
4. All new patients to be registered will need to schedule a one-hour "New Patient Visit" via the online scheduling program that is accessed by the direct link from this website's home page, or by Clicking Here. Children under 18 with non-eventful past medical histories may schedule a "Regular Office Visit" for the new patient visit.
5. PRIOR to the new patient visit, Dr. Speach must be made the patient's Primary Care Physician with the patient's insurance company. This may be done by calling the customer service number written on your insurance card. Note: Dr. Speach accepts Blue Cross, Blue Shield, Blue Choice , Preferred Care, and Aetna insurances only. Dr. Speach is not a Medicare Provider.
6. Each new patient should request a transfer of medical records from their previous physician. The appropriate Transfer of Records Formis available to download and print directly from this website. This form must be signed by the patient and delivered to the physician's office where the patient is transferring FROM. Patients should inquire of the cost of transferring records before submitting the form. If costs are prohibitive, you should contact Dr. Speach to decide which elements of the medical record are essential. Medical records received by Dr. Speach will be returned to the patient after electronic processing.
7. All patients should download, print, and sign the Signature Page Document and bring it to their New Patient Visit.
8. All patients should come to the new patient visit with all active prescription medications in hand, and prepared to discuss their past medical history in detail. Preparing before hand by completing the New Patient Medical History Survey in detail will help patients get more out of their first office visit.

Please Note: Dr. Speach's cell phone number should only be used in case of medical urgency and only by registered patients AFTER they have completed the new patient visit.
